How can I add categories to a custom page?

I have set up Zoom on my website, and it's working great. I'm looking to add it to a custom page http://pgi-tp.cee.uiuc.edu/forWeb/Technical_Files/list.asp I have the basic search set up, but would like to add the dropdown box for categories. I'm using ASP. Thank you for any help you can give.

The easiest way would be to go to your search page (http://pgi-tp.cee.uiuc.edu/forWeb/Zoom%20Search/search.asp) in a browser, select "View Source" from the menu, and locate the HTML for the categories dropdown. Then just copy and paste this to your custom search box on the other page.
